Uninsured Services & Annual Payments
Uninsured services are services that the Alberta government does not pay doctors to perform. In these cases, patients pay ‘out of pocket’. We will provide you with a receipt. (Please note: GST will be charged on top of stated fees).
Dr. Meghan Elkink is now offering two options to pay for uninsured services:
1. Pay as you go – payment per form throughout the year will be required.
2. Annual Payment – this fee covers most uninsured services you may need through the year.

*Please note: These services will typically start at the beginning of each calendar year (January 1st). As an added benefit this year, services purchased before January 1st, 2024 will cover up until January 1st, 2025.

Uninsured Services Fee Guide
School / Sports / Work/ Allied Health / Medical Fitness Forms
- $55 – Sick note, notice of fitness note, prescription for allied health (physiotherapy, massage therapy, chiropractic, acupuncture, etc.)
- $105 – Pregnancy leave and employment insurance form, seniors’ residence medical report, handicap placard form
- $145 – Notice of fitness form and exam
$350 – Pre-employment medical exam and report
$450 – Periodic industrial health exam, ministry of transport licenses exam and report
Insurance / AISH / Disability / Tax Credit Forms
- $250 -$500
Driver’s Medical
- $150 – Seniors driver’s medical
- $250 – Driver’s Medical
Clinical Services
- $100 – Treatment of warts (non-plantar)
- $200 – Uninsured office visit (regular appointment)
$250 – Uninsured office visit meet and greets, uninsured office visit annual check-up, missed appointment
Administrative Services
- $25+ – Transfer of medical chart to another physician’s office
- Fees Vary – Copy of medical records to a third party, medico-legal services
Additional Charges:
- Please note there may be additional charges based on:
- Additional time to complete forms or preparation time at $500/hr
- Rush services at 2x base service cost
